About This Course
Endodontic cases vary widely in complexity, and a significant proportion of high-complexity cases are encountered in general practice. The experience required, the chair-time involved and the risks of complication all rise with complexity — and misjudging that complexity before treatment begins is one of the most avoidable causes of poor outcomes.
This module teaches you to assess endodontic case complexity systematically, before you pick up an instrument. You will work through the clinical factors that make a case difficult, learn a structured classification system, and practise applying the Endodontic Complexity Assessment Tool (E-CAT) to real cases.
Endodontics accounts for around one in five dental negligence claims — the second most common reason for litigation. A thorough pre-assessment protects patients and practitioners alike. It also supports fairer use of resources: cases treated by practitioners of appropriate experience improve access to primary care, while specialist time is reserved for the cases that genuinely need it.
The module directly supports the General Dental Council’s Preparing for Practice learning outcomes 1.14.9 (recognising the risks of non-surgical root canal treatment and how to manage them) and 1.14.10 (evaluating the need for more complex treatment and referring accordingly), and GDC Standards principles 6 and 7.
What You Will Cover
- Module 1 — Why assess case complexity? The evidence base, the professional standards framework, and the benefits of appropriate case management.
- Module 2 — Assessing complexity. The classification system, worked through a fully explained example case, followed by a short pre-evaluation exercise.
- Module 3 — Complexity factors. The individual factors that raise difficulty, grouped into patient factors, the crown, and the root canal system — from medical history and radiographic difficulty through to root curvature, unique anatomy and previous endodontic treatment.
- Module 4 — Assessment tools. Classification tools in practice, including a guided demonstration of E-CAT.
- Final assessment. Fifteen clinical cases to assess independently, each with radiographs and clinical photographs.

By the end of this module you will be able to:
- Explain why pre-treatment complexity assessment matters clinically, professionally and medico-legally.
- Identify the patient, crown and root canal factors that increase endodontic treatment difficulty.
- Apply a structured classification system to categorise a case by complexity.
- Use E-CAT to produce a complexity score for a case.
- Judge whether a case sits within your own scope of practice, and refer appropriately when it does not.
Requirements
This module is designed for qualified dentists and dental students. A working knowledge of endodontic diagnosis and treatment is assumed; no other prior study is required.
You will need a device with a modern web browser. The assessment cases include radiographs and clinical photographs, so a reasonably sized screen is recommended. E-CAT opens in a separate browser tab, which some learners prefer to run on a second screen or device.
The module is self-paced. You may leave and return at any point, and your progress is saved.
